Notion Forms let you collect answers that land straight in a Notion database, one row per response. You can build a form on any plan, including Free. Conditional logic (show a question based on an earlier answer) needs the Business plan ($20 per member a month, billed yearly) or Enterprise. If you need payments, heavy file uploads or complex branching, a dedicated form tool is still better.
I use Notion Forms for content requests, bug reports and simple client intake. The big win is no glue. Before, I used Google Forms plus Zapier to push answers into Notion. Now the form is just a view of the database. Below is the setup, the sharing settings that matter, and the limits. What a Notion form is
A Notion form is a page people fill out. Each answer becomes a new row in a connected database. Each question is a database property, like Text, Select, Date or Person.
So the second someone hits Submit, the response can be sorted, filtered, assigned and picked up by your automations. A content team, for example, can point a request form at the same editorial calendar the writers already use.

People who fill out a form do not need a Notion seat. That makes forms a cheap way to collect input from clients or the public.
How to build a form, step by step
Option 1: start from scratch
- Open a page and type /form. Pick Form.
- Notion creates a new database behind it. It also adds a Respondent property that records who submitted.
- Add a title, a short description and, if you like, an icon or cover.
- Click each question to rename it, pick its type and turn on Required where needed.
- Use the preview to see what people will see.
Option 2: add a form to a database you already use
- Open the database. Click + next to the view tabs (or Add view in the view menu).
- Choose Form.
- Click Create {#} questions. Notion makes one question per property.
- Delete the questions you do not want people to answer, like Status or internal notes.
You need Full access to the database to create a form from it, and Can edit or Full access to edit one.
Writing good questions
- Rename questions in plain words. The property can stay “Req_Type.” The question should say “What do you need?”
- Use select options, not free text, for anything you will filter on later, like team or priority.
- Mark only true must-haves as required. Every extra required field cuts how many people finish.
- Keep it under 10 questions. If you need more, split it into two forms.
- Add help text under any question people might misread.
Conditional logic
On Business or Enterprise, you can show or skip questions based on an answer. Notion only supports logic on multiple-choice questions.
Example: a “Request type” question with Bug, Feature and Question. If someone picks Bug, show “Steps to reproduce” and “Browser.” If they pick Feature, jump to “What problem does this solve?”
Tips:
- Plan the branches on paper first.
- Keep it to one or two levels deep. Deep trees are hard to test.
- Test every path in preview before you share.
Sharing settings that matter
Click Share form, then choose Who can fill out:
- Anyone at your workspace with link. Members can fill it out. Guests need access to the database.
- Anyone on the web with link. Anyone can fill it out, even without a Notion account. Responses from outside your workspace are anonymous.
- No access. This closes the form.
Anonymous answers: you can let people stay anonymous in sharing settings. That is good for feedback surveys. It is bad for intake, where you need to reply.
Submit screen: under Form settings → Submit screen you can change the thank-you message.
Enterprise control: workspace owners can block forms from being shared to the web. Regulated teams should turn this on and only allow approved public forms.
After the response arrives
This is where Notion shines:
- Add a Status property with a default of “New.”
- Build a board view grouped by Status for triage.
- Use a database automation to notify a person or Slack channel when a new row is added. Database automations are on paid plans.
- Assign an owner and a due date as you triage.

My rule: if the data will be worked on inside Notion, use Notion Forms. If the form is a marketing page or takes payments, use a form tool.
Real examples that work well
- Content requests: title, audience, due date and a link. Lands on the editorial board.
- Bug reports: what happened, steps and a screenshot link. Lands in the engineering triage view.
- Event sign-ups: name, team and session choice. Lands in a table you can filter by session.
Common problems
- A guest cannot open the form. On workspace-only sharing, guests need database access. Switch to web sharing or grant access.
- Answers show “Anonymous.” That is expected for web forms from people outside your workspace.
- Logic option is missing. You are on Free or Plus, or the question is not multiple choice.
- Internal fields appear on the form. Remove those questions from the form. The properties stay in the database.
